The lymphodepletive and immunomodulatory effects of chemotherapy and radiation standard of care can be leveraged to enhance the antitumor efficacy of T cell immunotherapy. We outline a method for generating EGFRvIII-specific chimeric antigen receptor (CAR) T cells and administering them in the context of glioblastoma standard of care.
Adoptive T cell immunotherapy offers a promising strategy for specifically targeting and eliminating malignant gliomas. T cells can be engineered ex vivo to express chimeric antigen receptors specific for glioma antigens (CAR T cells). The expansion and function of adoptively transferred CAR T cells can be potentiated by the lymphodepletive and tumoricidal effects of standard of care chemotherapy and radiotherapy. We describe a method for generating CAR T cells targeting EGFRvIII, a glioma-specific antigen, and evaluating their efficacy when combined with a murine model of glioblastoma standard of care. T cells are engineered by transduction with a retroviral vector containing the anti-EGFRvIII CAR gene. Tumor-bearing animals are subjected to host conditioning by a course of temozolomide and whole brain irradiation at dose regimens designed to model clinical standard of care. CAR T cells are then delivered intravenously to primed hosts. This method can be used to evaluate the antitumor efficacy of CAR T cells in the context of standard of care.
Glioblastom (GBM) er den vanligste primære maligne hjernesvulst og er alltid dødelig. Kirurgisk reseksjon kombinert med ikke-spesifikk standardbehandling kjemoterapi og radioterapi unnlater å fullstendig eliminere maligne celler, noe som resulterer i en sturen prognose av mindre enn 15 måneder hos pasienter med denne sykdommen 1. I motsetning til dette, immunterapi gir en nøyaktig tilnærming for spesifikt å målrette tumorceller, og har således potensiale til å tjene som en meget effektiv behandling plattform med redusert risiko for sikkerhet toksisitet 2-4. Til tross for sin allsidighet, men den kliniske nytten av CAR adoptiv terapi har ikke blitt fullt ut realisert, delvis på grunn av tumorassosiert immunsuppresjon og immununndragelser 12-16 samt utfordringer i å etablere og opprettholde antigen-spesifikke T-celler in vivo. Utnytte standard of care (SOC) med immunterapi kan potensielt overvinne flere av disse limitasjoner, noe som resulterer i økt effekt i både preklinisk og klinisk setting.
SOC for post-reseksjon GBM består av høydose temozolomid (TMZ), en DNA alkyleringsmiddelet 17, og hele hjernen bestråling (WBI) 1. Disse behandlingene antas å virke synergistisk med kreftvaksiner via oppregulering av svulst MHC uttrykk 18-20 og utgytelsen av antigener av døde kreftceller 17,19,21,22. Faktisk tillegg av TMZ 20,23 eller WBI 18,24 fører til økt antitumor effekt av immunbaserte behandlinger i preklinisk innstillingen. Videre, som mange ikke-spesifikke cytotoksiske kjemoterapeutika, gis TMZ kjent for å forårsake systemisk lymfopeni 25,26, noe som kan utnyttes som et middel til host-condition for adoptiv terapi plattformer 27-29. TMZ-mediert lymphodepletion har vist seg å øke frekvensen og funksjon av antigen-spesifikke T-celler, noe som fører til økt effekt av en adoptive behandlingsplattform mot intrakranielle svulster 30. I sammenheng med CAR terapi, serverer lymphodepletion som et middel til host-condition ved både å redusere antall endogene suppressor T-celler 31, og indusere homeostatic spredning 32 via redusert konkurranse for cytokiner 33, og dermed forsterke antitumor aktivitet 11,34. Gitt den synergistisk forhold mellom GBM SOC og immunterapi plattformer, evaluere nye adoptiv terapier og vaksineplattformer i sammenheng med SOC er avgjørende for å trekke meningsfulle konklusjoner om effekt.
I denne protokoll har vi skissere en fremgangsmåte for generering og intravenøs administrering av murine EGFRvIII CAR-spesifikke T-celler sammen TMZ og WBI i mus som bærer EGFRvIII-positive intrakranielle tumorer (se figur 1 for behandling linjen). Kort fortalt er CAR T-celler gjort ex vivo ved retroviral transduksjon. Humane embryonale nyre (HEK) 293T-celler transfektert ved hjelp av en DNA / lipid-kompleks (som inneholder CAR vektor og PCL-Eco plasmider) for å produsere virus, som deretter brukes til å transdusere aktiverte murine splenocytter som er høstet og dyrket parallelt.
