Here we present a murine model of arteriovenous fistula (AVF) failure in which a clinically relevant anastomotic configuration is incorporated. This model can be used to study the pathophysiology and to test possible therapeutic interventions.
The arteriovenous fistula (AVF) still suffers from a high number of failures caused by insufficient remodeling and intimal hyperplasia from which the exact pathophysiology remains unknown. In order to unravel the pathophysiology a murine model of AVF-failure was developed in which the configuration of the anastomosis resembles the preferred situation in the clinical setting. A model was described in which an AVF is created by connecting the venous end of the branch of the external jugular vein to the side of the common carotid artery using interrupted sutures. At a histological level, we observed progressive stenotic intimal lesions in the venous outflow tract that is also seen in failed human AVFs. Although this procedure can be technically challenging due to the small dimensions of the animal, we were able to achieve a surgical success rate of 97% after sufficient training. The key advantage of a murine model is the availability of transgenic animals. In view of the different proposed mechanisms that are responsible for AVF failure, disabling genes that might play a role in vascular remodeling can help us to unravel the complex pathophysiology of AVF failure.
Een functionele vasculaire toegang conduit is van vitaal belang voor patiënten met nierfalen die afhankelijk zijn van chronische hemodialyse in leven te blijven. De bouw van een arterioveneuze fistel (AVF) is op dit moment de beste keuze voor vasculaire toegang. Echter, AVF complicaties vormen een belangrijke oorzaak van morbiditeit bij patiënten op chronische hemodialyse. Ondanks uitgebreid wetenschappelijke inspanningen, geen enkele nieuwe benaderingen AVF toegangsgerelateerde complicaties resulteerde in een substantiële verbetering van AVF duurzaamheid. Uit deze teleurstellende vooruitgang betreft onvolledige kennis van de onderliggende pathofysiologie van hemodialyse toegang falen.
Om de pathofysiologie van AV toegang falen, diermodellen dat de menselijke pathologie nauw nabootsen ontrafelen van het grootste belang. In dit verband naast de diersoort ook de anastomotische plek, de gewenste anti-coagulatie therapie en de duur van opvolging na surgery moet rekening worden gehouden met 1. Terwijl de grote dieren zijn het meest geschikt voor interventiestudies gericht op nieuwe therapeutische strategieën te ontwikkelen, muismodellen hebben het grootste potentieel om meer inzicht te krijgen in de moleculaire mechanismen die ten grondslag liggen AV toegang falen te krijgen als gevolg van de beschikbaarheid van transgene muizen. Bovendien kan een groot aantal muizen worden gebruikt voor dit doel tegen lagere kosten in vergelijking met grotere dierlijk gebruik.
De eerste muizenmodel van AVF falen is in 2004 beschreven door Kwei en et al. 2 In dit model werden AVFs geconstrueerd met behulp van de halsslagader en de halsader in een end-to-end wijze met een intravasculaire katheter. Dit model zou nuttig vroege veneuze aanpassingsmechanismen in AVFs bestuderen hoewel het end-to-end configuratie en de aanwezigheid van een intravasculaire katheter de geldigheid van dit model voor menselijke AVFs beperken. Een verbeterd AVF model werd geïntroduceerd door Castier en et al. 3 waarin het uiteinde vande halsslagader wordt verbonden met de zijkant van de halsader. Echter, AVFs bij hemodialysepatiënten zijn meestal vervaardigd door anatomizing eind een ader aan de zijkant van een slagader. De exacte configuratie van de AVF is een essentieel kenmerk van een AV toegangsmodel aangezien het hemodynamische profiel binnen het kanaal 4 bepaalt. Dit laatste is een belangrijke bijdrage aan dysfunctie en verdere ontwikkeling van intimale hyperplasie (IH) 5 endotheel.
Een nieuw muizenmodel werd onlangs ontwikkeld met dezelfde anatomische configuratie zoals gebruikt bij mensen 6. In dit model, AVF ontstaan in C57BL / 6 muizen door anastomose na een tak van de externe halsader aan de zijkant van de halsslagader met onderbroken hechtingen. In het onderhavige document, onderzoeken we de microchirurgische procedure van dit model teneinde het wijdverspreide gebruik van dit muismodel gericht vergemakkelijken de complexe pathofysiologie ontrafelenhemodialyse toegang mislukking.
