The chronically instrumented pregnant sheep has been used as a model of human fetal development and responses to pathophysiologic stimuli such as endotoxins, bacteria, umbilical cord occlusions, hypoxia and various pharmacological treatments. The life-saving clinical practices of glucocorticoid treatment in fetuses at risk of premature birth and the therapeutic hypothermia have been developed in this model. This is due to the unique amenability of the non-anesthetized fetal sheep to the surgical placement and maintenance of catheters and electrodes, allowing repetitive blood sampling, substance injection, recording of bioelectrical activity, application of electric stimulation and in vivo organ imaging. Here we describe the surgical instrumentation procedure required to achieve a stable chronically instrumented non-anesthetized fetal sheep model including characterization of the post-operative recovery from blood gas, metabolic and inflammation standpoints.

Den utstrakte bruken av fosterets sau modellen skyldes unique amenability av den ikke-bedøvet fetal sauene til kirurgisk plassering og opprettholdelse av kateteret og elektroder, noe som gir repeterende blodprøvetaking, opptak av bioelektriske aktivitet, anvendelse av elektrisk stimulering og in vivo avbildning av hjernen. 18 telemetri er også mulig, men mindre hyppig brukt men på grunn av høyere raffinement til å sette opp så vel som den første og vedlikeholdskostnader. 19
Videre er foster sauer modellen meget allsidig så mange varianter av instrumentering er mulig avhengig av hvilke tiltak av interesse. For eksempel er det mulig å ta opp over dager til uker multivariat signaler i sann tid som fosterpustebevegelser, elektrisk hjerneaktivitet, kardiovaskulære responser, elektrokardiogram, regional blodstrøm til en rekke organer ved hjelp av strømnings-prober eller mikrokuler, etc. Takket denne allsidigheten, et bredt spekter av studier er utført herunder utvikling av cardiovascular system 20,21, hypothalamo-hypofyse-binyre (HPA) aksen 22, hjernens utvikling 23 og søvntilstander utvikling spesielt 24, effekter av hypoksi / asfyksi 25, terapeutisk hypotermi 26, betennelse 6-11, kombinasjon av både 27, glukokortikoider 28,29, anti-depressiva 30, Bronkopulmonale dysplasi (BPD) 31,32, foster programmering 33,34,35,36,37,38,39 eller utvikling av nye fosterovervåking modaliteter før og under fødselen for å nevne få områder av etterforskningen. 40,41,42,43
Det overordnede målet med metoden presenteres er å vise denne allsidige modellens grunnleggende gjennomføring. Den tillater å etablere en rekke akutte og kroniske eksperimentelle protokoller som studerer føtalt fysiologi og patofysiologi på integrerende, orgel, cellulære og molekylære nivå.
