We developed an in vitro model of dormancy in the bone marrow for estrogen-sensitive breast cancer cells. The goal of this protocol is to demonstrate use of the model for the study of the molecular and cellular biology of dormancy and for generation of hypotheses for subsequent testing in vivo.
The study of breast cancer dormancy in the bone marrow is an exceptionally difficult undertaking due to the complexity of the interactions of dormant cells with their microenvironment, their rarity and the overwhelming excess of hematopoietic cells. Towards this end, we developed an in vitro 2D clonogenic model of dormancy of estrogen-sensitive breast cancer cells in the bone marrow. The model consists of a few key elements necessary for dormancy. These include 1) the use of estrogen sensitive breast cancer cells, which are the type likely to remain dormant for extended periods, 2) incubation of cells at clonogenic density, where the structural interaction of each cell is primarily with the substratum, 3) fibronectin, a key structural element of the marrow and 4) FGF-2, a growth factor abundantly synthesized by bone marrow stromal cells and heavily deposited in the extracellular matrix. Cells incubated with FGF-2 form dormant clones after 6 days, which consist of 12 or less cells that have a distinct flat appearance, are significantly larger and more spread out than growing cells and have large cytoplasm to nucleus ratios. In contrast, cells incubated without FGF-2 form primarily growing colonies consisting of >30 relatively small cells. Perturbations of the system with antibodies, inhibitors, peptides or nucleic acids on day 3 after incubation can significantly affect various phenotypic and molecular aspects of the dormant cells at 6 days and can be used to assess the roles of membrane-localized or intracellular molecules, factors or signaling pathways on the dormant state or survival of dormant cells. While recognizing the in vitro nature of the assay, it can function as a highly useful tool to glean significant information about the molecular mechanisms necessary for establishment and survival of dormant cells. This data can be used to generate hypotheses to be tested in vivo models.

Vissa micrometastases är tillväxt inletts, men återfall mönster beroende på celltyp. Patienter med trippel negativ bröstcancer tenderar att återkomma mellan 1 till 4 år, vilket tyder på dålig kontroll över vilande tillstånd. Andra celltyper, inklusive ER / PR + celler, kan förbli vilande i upp till 20 år, med en stadig, kontinuerlig andelen återfall 10. Även skillnader i dvala genuttryck signaturer mellan ER + och ER bröstcellinjer och tumörer speglar olika dvala potentialer 11, interaktioner med benmärgstroma utgör sannolikt ett betydande bidrag till dvala.
Studiet av dvala in vivo är exceptionellt svårt eftersom mikrometastaser är sällsynta och underlägsna av hematopoetiska celler med mer än 10 6-faldigt. Därför måste relevanta modeller skapas som ger in vitro-data som kan föreslå mekanismer och generera testbara hypoteser in vivo. Ett antal av dvala modeller, inklusive Mathe17,18 och spontana tumör och metastasering modellerna 19, har gett tiska modeller 12,13, in vitro modeller 7,8,14,15, in vivo xenograft-modeller 16, kombinationer av in vitro och xenograft-modeller en inblick i cancercell dvala 20 . Var och en av dessa modeller har sina egna begränsningar och är sig själva i första hand användas för att generera hypoteser om molekylär signalering och interaktioner som styr dvala som ska testas mer biologiskt relevanta modeller.
Med det övergripande målet att definiera de molekylära mekanismer dvala, samspelet med mikro som resulterar i cykelstopp, redifferentiering och terapeutiska motstånd och mekanismer som leder till återfall i ER + celler, vi utvecklat en in vitro-modell som ger utvalda relevanta delar av stromala mikromiljö 7. Denna modell, medan relativt gles i komponenter, är tillräckligt robust för att tillåta forskare att ta fram särskilda molekylära mekanismer som påverkar väsentliga funktioner dvala. Dessa experiment generera hypoteser som direkt kan testas in vivo. Modellen bygger på några viktiga faktorer som vi visat sig vara relevant i dvala. De inkluderar användningen av östrogenberoende bröstcancerceller, odling av cellerna vid en klonogen densitet där deras interaktion är i första hand med underlaget och lösliga komponenter i mediet, en fibronektin substrat och närvaron av basisk fibroblasttillväxtfaktor (FGF-2) i mediet.
Vi kännetecknas mekanismer som styr systemet in vitro, inklusive induktionen av cellcykelstopp av FGF-2 21, förmedlas genom TGFp 22, överlevnad signalering genom PI3-kinas 7,8 och ERK 8 och morfogena differentiering till en epitelial fenotyp, vilket berodde på RhoA inaktive, integrin45, 5β1 uppreglering och ligering av stromal fibronektin för överlevnad 7,15 (Figur 1). De cell in vitro cykel effekter av FGF-2 på MCF-7-celler börjar vid koncentrationer åtminstone en log under 10 ng / ml 21,23. Den logiska grunden var baserad på den tidsmässiga kontrollen av FGF-2 expressions reglerar bröst duktal morfogenes, cyklisk expansion och recession i ett antal däggdjurssystem 24-27. Vi visade att FGF-2 inducerar differentiering, inklusive duktal morfogenes i 3D kultur 28, och att FGF-2 uttryck i allmänhet förlorade med malign transformation av humana tumörer 29. Uttrycket av FGR1 förblev intakt i bröst tillfrågade 29 och MCF-7-celler karcinom fortsätter att uttrycka alla 4 FGF-receptorer 30. Inom ramen av dvala, är FGF-2 som exporteras av och kraftigt avsatt på benmärgstroma 31,32 där den fungerar i bevarandet av hematopoietiska stamceller 33. Vi DEMonstrated att FGF-2 inducerar ett vilande tillstånd i ER + bröstcancerceller odlade på fibronektin substrat, också rikligt i märgen, där det inducerar morfogena differentiering 7. I modellen, bröstcancerceller är tillväxt hämmas, inaktivera Rho A genom RhoGap GRAF, redifferentiate till en epitelial fenotyp och återuttryck integriner α5β1 lost med malign progression. De binder fibronektin genom integrin α5β1 och aktivera överlevnad signalerar att göra dem resistenta mot cytostatikabehandling 7,8,15 (Figur 1). Hämning av Rho klass GTPaser har visats tidigare att framkalla en vilande fenotyp 34.
Här kommer vi att beskriva de särskilda förfaranden som möjliggör utredare att fastställa modellen och undersöka specifika molekylära och cellulära mekanismer som styr dvala av ER + bröstcancerceller.
