The ability to model urinary tract infections (UTI) is crucial in order to be able to understand bacterial pathogenesis and spawn the development of novel therapeutics. This work’s goal is to demonstrate mouse models of experimental UTI and catheter associated UTI that recapitulate and predict findings seen in humans.
Urineweginfecties (UTI) een hoge prevalentie, een belangrijke oorzaak van morbiditeit en steeds resistent zijn tegen behandeling met antibiotica. Vrouwtjes worden onevenredig getroffen door UTI: 50% van alle vrouwen zal een UTI in hun leven hebben. Bovendien, 20-40% van de vrouwen die een eerste UTI zal een herhaling met enkele lijden frequente recidieven met een ernstige verslechtering van de kwaliteit van leven, pijn en ongemak, verstoring van de dagelijkse activiteiten te lijden, toegenomen kosten van de gezondheidszorg, en weinig behandelingsopties andere dan op lange termijn antibiotica profylaxe. Uropathogene Escherichia coli (UPEC) is de belangrijkste verwekker van de gemeenschap verworven UTI. Katheter geassocieerd UTI (Cauti) is de meest voorkomende ziekenhuis verworven infectie goed voor een miljoen exemplaren in de VS jaarlijks en dramatische kosten van de gezondheidszorg. Terwijl UPEC is de primaire oorzaak van Cauti andere veroorzakers zijn van toegenomen belang zoals Enterococcusfaecalis. Hier gebruiken we twee gevestigde muismodellen dat veel van de klinische karakteristieken van deze menselijke ziekten recapituleren. Voor UTI, een C3H / HeN model recapituleert veel van de functies van UPEC virulentie waargenomen bij de mens, waaronder gastheer reacties, IBC vorming en filamentatie. Voor Cauti een model gebruikt C57BL / 6 muizen, die catheter blaas implantaten behouden, is gevoelig te zijn E. faecalis blaasontsteking. Deze representatieve modellen worden gebruikt om opvallende nieuwe inzichten in de pathogenese van de ziekte UTI, wat leidt tot de ontwikkeling van nieuwe therapeutische en beheer of preventiestrategieën.
Urineweginfecties (UWI) zijn één van de meest voorkomende bacteriële infecties en kan worden onderverdeeld in twee categorieën op basis van het mechanisme van de overname, de gemeenschap en nosocomiale verworven UTI. Gemeenschap verworven UTIs vaak voor bij gezonde vrouwen en studies hebben aangetoond dat ongeveer 50% van de vrouwen minstens één UTI in hun leven 1. Bovendien herhaling is een groot probleem. Een vrouw die een eerste acute infectie heeft, heeft een 25-40% kans op een tweede infectie binnen zes maanden, ondanks de juiste behandeling met antibiotica en veel vrouwen blijven frequente recidieven 2 hebben. De bacteriën die deze infecties worden steeds antibioticaresistente verdere verstorende behandelingsprotocollen 3-6. UTI invloed miljoenen mensen per jaar kost ongeveer 2,5 miljard dollar in de gezondheidszorg gerelateerde kosten in de VS, het benadrukken van de impact en de prevalentie van de ziekte1,7 .Nosocomial verworven urineweginfecties zijn vooral geassocieerd met de aanwezigheid van buitenlandse instanties, zoals inwonende katheters. Katheter geassocieerde urineweginfecties (Cauti) blijven de meest voorkomende nosocomiale UTI verworven, goed voor ~ 70-80% van dergelijke infecties 8. Bovendien Cauti is geassocieerd met verhoogde morbiditeit en mortaliteit en het is de meest voorkomende oorzaak van secundaire infecties bloedbaan 9.
UPEC geassocieerd gemeenschap verworven urineweginfecties zijn gedacht te worden veroorzaakt door de introductie van bacteriën in de blaas van reservoirs in het maagdarmkanaal via mechanische manipulatie tijdens de geslachtsgemeenschap, slechte hygiëne of andere microbiële populatiedynamiek tussen verschillende gastheer niches 10. Eenmaal in de blaas, UPEC gebruik talrijke virulentiefactoren, zoals capsule, iron acquisition systems, toxinen, een virulentie plasmide, tRNAs, pathogeniteit eilanden en kolonisatie factoren waarvan is aangetoond een rol te spelen in de pathogenese <sup> 11-14. Van cruciaal belang voor de oprichting van UPEC kolonisatie, UPEC ook coderen meerdere soorten lijm chaperone Usher pathway (CUP) pili dat receptoren met stereo- specificiteit 15 herkennen. Type 1 pili, gaven de adhesine FimH, worden uitgedrukt door UPEC en binden gemannosyleerd uroplakins 16 en α-1, β-3 integrines 17, die zijn uitgedrukt op het luminale oppervlak van zowel mens en muis blazen 18. Deze FimH-gemedieerde interacties bevorderen bacteriële kolonisatie en invasie van de oppervlakkige epitheelcellen 19,20. Eenmaal in de cel kan UPEC ontsnappen in het cytoplasma, waar een bacterie snel kan verdelen om een intracellulaire bacteriële gemeenschappen (IBC), die bij rijping kan 10 4 bacteriën bevatten 21 ~ vormen. IBC formatie is aangetoond in ten minste zes verschillende muizenstammen, C3H / HeN, C3H / HeJ, C57BL / 6, CBA, FVB / NJ en BALB / c, en met een groot aantal verschillende UPEC stammen en andere Enterobacteriaceae 22-24. Maar in tijd en ruimte verschillen IBC vorming kan variëren afhankelijk van de muis achtergrond en de infecterende UPEC-stam. In C3H / HeN muizen geïnfecteerd met de prototypische UPEC stammen UTI89 of CFT073, IBC formatie kan worden gevisualiseerd als kleine biomassa bacteriën zo vroeg als 3 hpi (uur na infectie). Deze gemeenschap blijft groeien en bereikt een "middelpunt" van het complex op ongeveer 6 HPI wanneer de staafvormige bacterie nemen een groot percentage van de cytoplasmatische ruimte van terminaal gedifferentieerde cellen oppervlakkige paraplu Deze vroege IBC vorm relatief synchrone wijze met de meeste vergelijkbare afmetingen tonen en morfologie. ~ 8 hpi de bacteriën in de IBC overgang van een bacillen morfologie kokken. IBC's zijn van voorbijgaande aard. Aldus IBC rijping 12-18 HPI resultaten in verdere expansie van de bacteriële populatie, gevolgd door hun filamentatie en dispersie uit de cel with daaropvolgende verspreiding naar naburige cellen 23. Zo is de IBC niche zorgt voor een snelle bacteriegroei in een omgeving beschermd tegen gastheer immuunreacties en antibiotica 25. De verschillende stadia van UPEC infectie die wordt waargenomen bij muizen zijn ook waargenomen bij mensen, zoals IBC en filamentatie, ondersteunen het muizenmodel van UTI als een voordelig instrument dat kan worden gemodelleerd UTI bij mensen 22,26-28.
Terwijl een meerderheid van de vrouwen ervaren een UTI in hun leven, kan de uitkomst van deze infecties variëren van acute zelfbeperkende infectie met geen recidief, frequent terugkerende blaasontsteking. Verder hebben studies aangetoond een sterk familiair voorkomen van UTI, hetgeen duidt op een genetische component bijdraagt aan UTI gevoeligheid 29. We hebben ontdekt dat de verschillende UTI uitkomsten gezien in klinieken kan worden weerspiegeld door de verschillende uitkomsten van experimentele UPEC infectie onder inteelt muizenstammen 30. Bijvoorbeeld, C3H / HeN, CBA, DBA, en C3H / HeOuJ muizen gevoelig, in een infectueuze dosis-afhankelijke manier, om langdurige, chronische cystitis gekenmerkt door aanhoudende, hoge titer bacteriën (> 10 4 kolonievormende eenheden (CFU) / ml), hoge titer bacteriële blaas lasten op offer> 4 weken na infectie (WPI), chronische ontsteking, en urotheliale necrose. Deze muizen ook worden weergegeven verhoogde serumniveaus van IL-6, G-CSF, KC en IL-5 in de eerste 24 HPI die dienen als biomarkers voor de ontwikkeling van chronische cystitis. Dit kan goed mogelijk de natuurlijke loop van UTI vertegenwoordigen bij sommige vrouwen, placebo studies hebben aangetoond dat een groot percentage van de vrouwen die UTI zal hoge bacteriën in hun urine handhaven gedurende enkele weken na de eerste symptomen van blaasontsteking indien niet gegeven antibiotica 31 , 32. Verder gebruik C3H / HeN muizen, vonden we dat een geschiedenis van chronische cystitis is een belangrijke risicofactor voor de volgende ernstige terugkerende infecties. Terugkerende UTI is de meest significant klinische manifestatie van UTI en de C3H / HeN muis is momenteel de enige bestudeerde model dat een verhoogde aanleg na eerdere blootstelling recapituleert. Een tweede UTI uitkomst wordt gerecapituleerd in C57BL / 6 muizen, waar acute UPEC infectie zelfbeperkend, met een resolutie van de blaas ontsteking en bacteriurie binnen ongeveer een week. Interessant is dat in dit model, UPEC vormen gemakkelijk rustende intracellulaire reservoirs binnen de blaas weefsel waarvan UPEC in staat zijn die uit een slapende toestand naar een actieve UTI hervatten, eventueel verklaren één mechanisme voor dezelfde stam terugkerende UTI bij mensen 33, 34.
Naast de genetische invloeden op UTI gevoeligheid, invoering van een katheter in de blaas verhoogt de kans op een infectie en verhoogt het bereik van bacteriën in staat om een infectie te veroorzaken. Er is aangetoond dat menselijke Blaaskatheterisatie veroorzaakt histologische enimmunologische veranderingen in de blaas door mechanische stress die resulteert in een sterke ontstekingsreactie, afschilfering, oedeem van de lamina propria en submucusa, urotheel dunner en mucosale laesie van het urotheel en nieren 35,36. Bovendien, de katheter een oppervlaktebescherming voor bacteriële hechting zo een omgeving gebruiken Verscheidene soorten Cauti veroorzaken creëren. Terwijl UPEC zijn nog steeds een belangrijke bijdrage, Enterococcus faecalis is goed voor 15% van deze Cauti 37. Tijdens Blaaskatheterisatie is de gastheer gevoelig voor microbiële hechting, vermenigvuldigen en verspreiden in de urinewegen 39,40. E. faecalis vormen een biofilm op de katheter als onderdeel van een mechanisme te volharden in de blaas en verspreidt onder de nieren, is weergegeven in een muis 41 model Cauti. Recentelijk is aangetoond tijdens Blaaskatheterisatie wordt fibrinogeen (Fg) vrij in de blaas als onderdeel van de ontstekingsreactie. Fg accumuleert in de blaas, bedekt de catheter en is essentieel voor E. faecalis biofilm vorming, functioneren als een bijlage schavot. In een C57BL / 6 muis-model van Cauti, dat ontdekten we E. faecalis biofilm vorming op de katheter, en derhalve persistentie in de blaas, afhankelijk was van de Ebp pilus specifiek zijn punt adhesine EbpA. We vonden dat het N-terminale domein van EbpA specifiek bindt aan FG bekleden van de katheter. Bovendien werd gevonden dat E. faecalis gebruikt Fg als metaboliet bron tijdens infectie, waardoor het verbeteren biofilmvorming 42.
Muismodellen hebben kritische bewezen understanding en voorspellen van klinische manifestaties van UTI en Cauti 41. In dit artikel tonen we inoculum voorbereiding van de blaasontsteking UPEC isoleren UTI89 en transurethrale inoculatie van C3H / HeN muizen. Verder tonen we een protocol voor catheter inbrengen in C57BL / 6 muizen en inoculatie van de E. faecalis OG1RF stam. Beide technieken leiden tot consistente en betrouwbare UTI of Cauti bij muizen. We geven ook de technieken die worden gebruikt om de vorming van IBC observeren tijdens acute blaasontsteking en urine verzamelen voor de analyse van chronische of recidiverende blaasontsteking. C3H / HeN-muizen werden gebruikt om aspecten van UPEC pathogenese waaronder initiële bacteriële invasie, IBC vorming filamentatie en de ontwikkeling van chronische blaasontsteking 23,33,43 bestuderen. Deze virulentie parameters zijn ook onderzocht bij een verscheidenheid van andere muizen achtergronden 22,33. Voor Cauti, de C57BL / 6-model zorgt voor een vreemd lichaam implantatie in de blaas met daaropvolgende bacteriële colonization, die kan worden gehandhaafd gedurende 7 dagen na infectie 41. Deze modellen zijn nuttig voor de beoordeling van bacteriële virulentie mechanismen, gastheer reacties op UTI en mechanismen om gastheer reacties, waarvan een groot deel is vervolgens recapituleerde of waargenomen in klinische menselijke bevolking ondermijnen geweest.
Ongecompliceerde gemeenschap verworven UTI is een veel voorkomende en kostbare infectie goed voor enkele miljoenen eerstelijns gezondheidszorg bezoeken per jaar 46. Daarnaast Cautis zijn een gemeenschappelijke zorg verworven infectie die zeer kostbaar om zorgverleners als de Centra van Medicare en Medicaid Services is geworden niet meer vergoedt providers voor de extra kosten van de behandeling als gevolg van het ziekenhuis verworven Cauti 45.
