The cuff technique shortens and facilitates the model of heterotopic cervical heart transplantation in mice by avoiding technically challenging suture anastomoses of small vessels. The technical details shown in this video paper should allow researches to establish this model in their laboratories.
The heterotopic cervical heart transplantation in mice is a valuable tool in transplant and cardiovascular research. The cuff technique greatly simplifies this model by avoiding challenging suture anastomoses of small vessels thereby reducing warm ischemia time. In comparison to abdominal graft implantation the cervical model is less invasive and the implanted graft is easily accessible for further follow-up examinations. Anastomoses are performed by pulling the ascending aorta of the graft over the cuff with the recipient’s common carotid artery and by pulling the main pulmonary artery over the cuff with the external jugular vein. Selection of appropriate cuff size and complete mobilization of the vessels are important for successful revascularization. Ischemia-reperfusion (I/R) injury can be minimized by perfusing the graft with a cardioplegic solution and by hypothermia. In this article, we provide technical details for a simplified and improved cuff technique, which should allow surgeons with basic microsurgical skills to perform the procedure with a high success rate.
Hjertet transplantation modellen bruges ofte til at undersøge I / R-skade, allograftafstødning, efter transplantation vaskulopati og effektiviteten af immunmodulerende midler. Musemodeller kombinerer mange fordele som den kendte immunologiske og genetiske baggrunde, tilgængeligheden af mange indavlede og transgene stammer og relativt lave eksperimentelle omkostninger.
I 1973 blev en teknik af abdominal hjertetransplantation hos mus først beskrevet af Corry et al 1. I denne model podninger er revaskulariseret af end-to-side anastomoser af de opstigende aorta til modtagerens abdominale aorta og i de vigtigste lungepulsåren til vena cava inferior. I 1991 blev en sutur teknik til livmoderhalskræft hjertetransplantation i mus beskrevet af Chen et al 2. I denne model en retrograd koronararterie perfusion etableres ved anastomose af modtagerens halspulsåre til de opstigende aorta GRagter. Veneblodet afløb via koronar sinus ind i højre atrium, højre hjertekammer og udstødes gennem lungepulsåren i recipienten eksterne halsvene (figur 1). I sammenligning med den abdominale graft implantation cervikal model er mindre invasiv og overfladiske placering af implantatet giver en meget nem tilgængelighed for yderligere opfølgende undersøgelser (f.eks palpering, ekkokardiografi, intravital fluorescensmikroskopi) 3,4.
Trods tekniske forbedringer 5,6, har en mere udbredt anvendelse af denne model blevet begrænset på grund af tekniske vanskeligheder at udføre syet anastomoser af små fartøjer med en høj forekomst af anastomose lækage og trombose. I 1991 Matsuura et al. indført manchetten teknik, hvor karvæggen krænges over et syntetisk cylinder, hvilket i høj grad letter model af cervikal hjertetransplantation 7. Teknikken var further vedtaget for forskellige eksperimentelle transplanterede modeller, herunder renal 8, pancreas 9, led 10, hepatisk 11,12, lunge 13,14 og vaskulær transplantation 15. I sammenligning med suturen teknik krævede tid for anastomoser og dermed den varme iskæmi er kortere, og der er betydeligt mindre vaskulære komplikationer ved hjælp af manchetten teknik 16. Desuden giver kirurger med lidt træning i mikrokirurgi til at udføre operationen med en høj succesrate. En ulempe ved manchetten teknik er forudsætningen ligering af arteria carotis communis, hvilket kan ændre cerebral perfusion.
I denne video afhandling præsenterer vi en forbedret og forenklet manchet teknik til livmoderhalskræft hjertetransplantation i mus.
Vi har gennemført over 200 hjerte-transplantationer med en succesrate på over 90% i forskellige eksperimentelle indstillinger. Med teknikken beskrevet her, kan den fuldstændige procedure ske inden for 60 minutter af en erfaren kirurg. Ved udarbejdelsen af cervikale skibe før indkøb orgel, kan den samlede graft iskæmi tid være begrænset til 20 minutter. Tid til implantation (varm iskæmi tid) er væsentlig kortere hjælp af manchetten teknik i forhold til suturen teknik 16.
