Denne protokollen presenterer en robust, reproduserbar modell av vaskularisert kompositt allotransplant (VCA) rettet mot samtidig studie av immunologi og funksjonell utvinning. Lemtransplantasjon spesielt og vaskularisert kompositt allotransplant (VCA) generelt har bredt terapeutisk løfte som har blitt stymied av gjeldende begrensninger i immunsuppresjon og funksjonell nevromotorisk utvinning. Mange dyremodeller er utviklet for å studere unike egenskaper ved VCA, men her presenterer vi en robust reproduserbar modell av ortotopisk bakbenstransplantasjon hos rotter designet for å samtidig undersøke begge aspektene av gjeldende VCA-begrensning: immunsuppresjonsstrategier og funksjonell nevromotorisk gjenoppretting. Kjernen i modellen hviler en forpliktelse til grundige, tidstestede mikrokirurgiske teknikker som håndsydd vaskulære anastomoser og håndsydd nevrale coaptation av lårnerven og isjiasnerven. Denne tilnærmingen gir holdbare lemrekonstruksjoner som gjør det mulig for lengre levende dyr som er i stand til rehabilitering, gjenopptakelse av daglige aktiviteter og funksjonell testing. Med kortvarig behandling av konvensjonelle immunsuppressive midler overlevde allotransplantede dyr opptil 70 dager etter transplantasjon, og isotransplanterte dyr gir langvarige kontroller utover 200 dager etter operativt. Bevis på nevrologisk funksjonell gjenoppretting er tilstede innen 30 dager etter operativt. Denne modellen gir ikke bare en nyttig plattform for avhør immunologiske spørsmål som er unike for VCA og nerveregenerering, men gir også mulighet for in vivo-testing av nye terapeutiske strategier spesielt skreddersydd for VCA.
Lemtransplantasjon under den bredere kategorien av vaskularisert kompositt allotransplant (VCA) eller komposittvev allotransplant (CTA) har ennå ikke oppfylt sitt terapeutiske løfte. Siden de første vellykkede menneskelige håndtransplantasjoner i Lyon, Frankrike og Louisville, Kentucky i 1998 og 1999, over 100 øvre ekstremiteter transplantasjoner har blitt utført over hele verden i nøye utvalgtepasienter 1. Større anvendelighet har blitt hindret av betydelig immunsuppresjon og begrenset funksjonell nevromotorisk gjenoppretting. Nåværende immunsuppresjonsstrategier resulterer i 85% forekomst av akutt avvisning i møte med 77% forekomst av opportunistisk infeksjon2. På den annen side oppstår funksjonell gjenoppretting etter håndtransplantasjon; gjennomsnittlig funksjonshemming av arm skulder og hånd (DASH) score forbedre fra 71 til 43, men at funksjonsnivået kan fortsatt kvalifisere som enfunksjonshemming 2. Gitt nonlife redde natur lem transplantasjon, gjeldende teknikker må raffineres i dyremodeller for å ta neste skritt i VCA.
Siden den første rottemodellen av lemtransplantasjon i 19783,har mange innovative dyremodeller blitt utviklet for å fremme feltet VCA4, som omfatter vaskulære mansjett anastomoser for å minimere operativ tid5,6, heteroto osteomyokutane transplantasjoner for å minimere fysiologisk fornærmelse mot mottakerdyret7,8,9,,10,,11og nye immunologiske tilnærminger7,12,13,14. Rottemodellen av ortotopiske høyre bakben midt på lårtransplantasjonen som presenteres her, understreker omhyggelige, tidstestede mikrokirurgiske teknikker som håndsydd vaskulære anastomoser og nevrale coaptation som en forhåndsinvestering i en robust, reproduserbar modellplattform for samtidig å undersøke begge aspektene av gjeldende VCA-begrensning: immunsuppresjonsstrategier og funksjonell nevromotorisk gjenoppretting.
Lemtransplantasjon, under den bredere kategorien av vaskularisert komponent allotransplantasjon (VCA), har allment anvendelig terapeutisk løfte som ennå ikke oppfylt. De viktigste veisperringene ligger i uløste immunologiske problemer som er unike for VCA og nevromotoriske utvinningsteknikker som brukes for tiden.
