Dit artikel beschrijft de toepassing van geautomatiseerde apparatuur om stoffen, zoals celvrij DNA en circulerende tumorcellen, eenvoudig en efficiënt te scheiden en te verzamelen uit volbloed.
Onlangs zijn vloeibare biopsieën gebruikt om verschillende ziekten te diagnosticeren, waaronder kanker. Lichaamsvloeistoffen bevatten veel stoffen, waaronder cellen, eiwitten en nucleïnezuren afkomstig uit normale weefsels, maar sommige van deze stoffen zijn ook afkomstig uit het zieke gebied. Het onderzoek en de analyse van deze stoffen in de lichaamsvloeistoffen spelen een cruciale rol bij de diagnose van verschillende ziekten. Daarom is het belangrijk om de vereiste stoffen nauwkeurig te scheiden en worden verschillende technieken ontwikkeld om voor dit doel te worden gebruikt.
We hebben een lab-on-a-disc type apparaat en platform ontwikkeld met de naam CD-PRIME. Dit apparaat is geautomatiseerd en heeft goede resultaten voor monstervervuiling en monsterstabiliteit. Bovendien heeft het voordelen van een goede acquisitieopbrengst, een korte bedrijfstijd en een hoge reproduceerbaarheid. Bovendien kunnen, afhankelijk van het type schijf dat moet worden gemonteerd, plasma met celvrij DNA, circulerende tumorcellen, perifere mononucleaire bloedcellen of buffy-lagen worden gescheiden. Zo kan de verwerving van een verscheidenheid aan materialen die aanwezig zijn in de lichaamsvloeistoffen worden gedaan voor een verscheidenheid aan downstream-toepassingen, waaronder de studie van omics.
Vroege en nauwkeurige detectie van verschillende ziekten, waaronder kanker, is de belangrijkste factor bij het vaststellen van een behandelingsstrategie 1,2,3,4. Met name vroege opsporing van kanker hangt nauw samen met verhoogde overlevingskansen voor de patiënt 5,6,7,8. Onlangs hebben vloeibare biopsieën in de schijnwerpers gestaan voor de vroege opsporing van kanker. Solide tumoren ondergaan angiogenese en geven verschillende stoffen af aan het bloed. Met name circulerende DNA’s (ctDNA’s), circulerende RNA’s (ctRNA’s), eiwitten, blaasjes zoals exosomen en circulerende tumorcellen (CTC’s) zijn gevonden in het bloed van kankerpatiënten 2,9. Hoewel er verschillen zijn in de hoeveelheid van deze stoffen, worden ze consequent waargenomen, niet alleen in de vroege stadia, maar ook in de latere stadia 6,10. Deze individuele verschillen zijn echter erg groot; de hoeveelheid celvrij DNA (cfDNA) dat ctDNA bevat, is bijvoorbeeld minder dan 1.000 ng en het aantal CTC’s is minder dan 100 in 10 ml volbloed van kankerpatiënten 11,12,13. Veel studies hebben kanker gekarakteriseerd met behulp van deze stoffen die in kleinere hoeveelheden aanwezig zijn (d.w.z. cfDNA, ctDNA en CTC’s). Om nauwkeurige resultaten te verkrijgen, is het belangrijk om kleine hoeveelheden stoffen met een hoge zuiverheid13,14 nauwkeurig te scheiden. Conventionele centrifugatiemethoden worden vaak gebruikt, maar ze zijn moeilijk te hanteren en hebben een lage zuiverheid, afhankelijk van de vaardigheid van de gebruiker. Sinds de ontdekking van CTC’s zijn verschillende scheidingstechnieken ontwikkeld, zoals centrifugatie of scheiding van dichtheidsgraad, immunokraal en microfluïdische methoden. Sinds de ontdekking van CTC’s zijn er verschillende containmenttechnieken ontwikkeld. Deze technieken zijn echter vaak beperkt wanneer het nodig is om cellen te isoleren van de verschillende chips en membranen die worden gebruikt om ze te isoleren15. Ook vereisen de tagging-methoden apparatuur zoals FACS en zijn er grenzen aan het downstream-proces als gevolg van tagging-verontreiniging.
Onlangs is het gebruik van vloeibare biopsieën toegenomen en worden er verschillende onderzoeken uitgevoerd voor de vroege opsporing van kanker. Hoewel deze methode eenvoudig is, zijn er nog steeds moeilijkheden bij downstream-analyse en verschillende studies proberen deze moeilijkheden te overwinnen 16,17. Bovendien vereisen veel sites, waaronder ziekenhuizen, geautomatiseerde, reproduceerbare en zeer zuivere methoden die gemakkelijk te gebruiken zijn. Hier hebben we een lab-on-a-disc ontwikkeld voor de geautomatiseerde scheiding van stoffen uit bloedmonsters na een vloeibare biopsie. Deze apparaten zijn gebaseerd op het principe van centrifugatie, microfluïdica en celopname ter grootte van een porie. Er zijn drie soorten schijven: LBx-1 kan plasma en buffy coat verkrijgen, terwijl LBx-2 plasma en PBMC kan verkrijgen uit volbloed met een volume van minder dan 10 ml; FAST-auto kan ook CTC’s verkrijgen met behulp van een membraan dat van de schijf kan worden verwijderd. Maximaal vier van elke schijf kunnen in één keer worden gebruikt. Bovenal is het voordeel van dit apparaat en deze methode dat het een verscheidenheid aan van kanker afgeleide stoffen uit hetzelfde monster kan verkrijgen met behulp van een kleine hoeveelheid bloed. Dit betekent dat het bloed van de patiënt slechts één keer hoeft te worden afgenomen. Bovendien heeft het het voordeel dat fouten als gevolg van verschillen in de bloedafnameperiode worden uitgesloten. Dit platform is eenvoudig te gebruiken en biedt nauwkeurige resultaten voor vloeibare biopsieën en downstream-toepassingen. In dit protocol wordt het gebruik van het apparaat en de cartridge geïntroduceerd.
De hoeveelheid en concentratie van cfDNA en CTC hangt af van het individu, het stadium en het type kanker. Het hangt ook af van de toestand van de patiënt 2,4,5,10,20. Met name in de vroege of precancereuze stadia van kanker zijn de concentraties van kankergerelateerde stoffen erg laag, dus er is een grote kans dat het niet kan worden gedetecteerd. Niettemin …
