The goal of this protocol is to build up a three-dimensional full thickness skin equivalent, which resembles natural skin. With a specifically constructed automated wounding device, precise and reproducible wounds can be generated under maintenance of sterility.
In vitro models are a cost effective and ethical alternative to study cutaneous wound healing processes. Moreover, by using human cells, these models reflect the human wound situation better than animal models. Although two-dimensional models are widely used to investigate processes such as cellular migration and proliferation, models that are more complex are required to gain a deeper knowledge about wound healing. Besides a suitable model system, the generation of precise and reproducible wounds is crucial to ensure comparable results between different test runs. In this study, the generation of a three-dimensional full thickness skin equivalent to study wound healing is shown. The dermal part of the models is comprised of human dermal fibroblast embedded in a rat-tail collagen type I hydrogel. Following the inoculation with human epidermal keratinocytes and consequent culture at the air-liquid interface, a multilayered epidermis is formed on top of the models. To study the wound healing process, we additionally developed an automated wounding device, which generates standardized wounds in a sterile atmosphere.
De huid is het grootste orgaan van het lichaam. Het creëert een barrière tussen de externe omgeving en de interne organen. Bovendien is de huid beschermt het lichaam tegen vochtverlies, milieu-invloeden, verwondingen en infecties en helpt om de lichaamstemperatuur 1 reguleren. Door zijn open ligging, wordt de huid vaak beïnvloed door mechanische, thermische of chemische trauma. Hoewel de huid is over het algemeen in staat zichzelf te repareren, kan meerdere lokale factoren zoals infectie, zuurstof, en veneuze toereikendheid leiden tot verstoorde wondgenezing. Wondgenezing kan worden gestoord door systemische factoren zoals obesitas, alcoholisme, roken, medicatie, voeding en ziekten zoals diabetes. 2
Het proces van wondgenezing kan worden verdeeld in 3 fasen: (i) de ontstekingsfase, (ii) de proliferatieve en (iii) remodelleringsfase. Bij schade aan de huid, een complex signaal cascade begint, waardoor de sluiting van de wond. 3Na een blessure, wordt de wond bloeden en een bloedstolsel wordt gevormd. Fibroblasten verhuizen naar het bloedstolsel en vervang het door nieuw weefsel dat vervolgens wordt verbouwd door de jaren heen.
Het huidige begrip van de biologische processen onderliggende huid herstel beperkt. Kleine dieren en varken modellen zijn gebruikt om wondgenezing te bestuderen. Echter, deze resultaten niet direct worden overgedragen op de mens als gevolg van species-specifieke verschillen. Naast deze in vivo modellen, kunnen bepaalde aspecten van wondgenezing worden bestudeerd door een gewikkelde toestand gesimuleerd door krassen in vitro monolaag kweken gebaseerd op geïmmortaliseerde cellijnen of primaire cellen. 4 Deze krassen modellen zijn zeer gestandaardiseerd, maar onvoldoende weerspiegelen de in vivo fysiologie. 5 Naast tweedimensionale modellen, zijn driedimensionale humane huid equivalenten ontwikkeld voor dermatologisch onderzoek. De dermale deel van deze modellen zijn gevergoede met behulp van diverse steigers inclusief gedecellulariseerde dermis, 6 collageen hydrogels, 7,8 glycosaminoglycanen 9 of synthetische materialen. 10 Gebruikmakend van deze huid equivalenten, de rol van epitheliale-mesenchymale interacties 11, de re-epithelialisatie, de cellulaire overspraak tussen fibroblasten en keratinocyten en de invloed van verschillende groeifactoren kunnen worden bestudeerd. Bovendien zijn deze modellen zijn nuttig om nieuwe kennis over hoe fibroblasten migreren naar de gewonde gebied en hoe chemotactische factoren beïnvloeden weefselregeneratie te krijgen. 12
Niet alleen genereren van de wondgenezing model zelf is uitdagend, maar ook om vast te stellen een zeer gestandaardiseerde wond model problematisch. Gebruikte technieken om te creëren wonden zijn krastesten, 13 brandwonden, 14 tape slijtage, 15 thermisch letsel, 16 zuig blaren, 17 vloeibare stikstof, 18 lasers, 19 </sup> scalpels, 18 meshers 6 en biopsie stoten. 20 De meeste van deze methoden hebben dezelfde valkuilen. Blessures handmatig geïmplementeerd zijn moeilijk te standaardiseren en te reproduceren tussen meerdere tests. Grootte, vorm en diepte van de wond variëren tussen de studies en daarmee afbreuk doen aan de kwaliteit van de onderzoeksgegevens. Het gebruik van laser voor gedefinieerde huid verwonden kan relatief eenvoudig worden gestandaardiseerd, maar leidt tot een situatie nabootsen van brandwonden. Heat van laser toegepast kan eiwitdenaturatie, bloedplaatjes aggregatie of vaartuigen vernauwing, wat kan leiden tot necrotisch weefsel.
In een alternatieve benadering, een geautomatiseerde verwonding apparaat (AWD), die ons in staat stelt om te definiëren en de huidwonden onder steriele omstandigheden te genereren ontwikkelden we. Verwonding parameters, zoals diepte en snelheid van penetratie en toerental van de boorkop kan worden geregeld. In deze studie hebben we gecombineerd de AWD met in eigen huis ontwikkelde volledige dikte van de huid equivalenten (ftSE) die vergelijkbaar zijn met een protocol gepubliceerd door Gangatirkar et al. 8 de dermale laag van de huid equivalent uit humane dermale fibroblasten (HDF), die zijn ingebed in een collageen I hydrogel. Op de huid laag, humane epidermale keratinocyten (HEK) zijn gezaaid. Binnen twee weken aan de lucht-vloeistof-interface van de HEK opbouwen van een epidermis bestaat uit meerdere vitale cel lagen en een hoornlaag. Naast de generatie van dit model, deze studie toont het gebruik van de AWD te definiëren en de wonden te creëren in de FTSE.
In vitro cellen gewoonlijk uitgebreid tweedimensionale celculturen, waarbij cellen hechten aan kunststof oppervlakken. Echter, deze kweekomstandigheden niet de fysiologische driedimensionale omstandigheden waarbij cellen groeien in vivo. Onder driedimensionale omstandigheden kunnen cellen natuurlijke cel-cel en cel-matrix aanhangsels samen en migreren in drie dimensies.
