This protocol describes the feasibility to perform miRNA profiling in exosomes, released in plasma of NSCLC patients, through a commercial exosome isolation kit with Proteinase K and RNAse treatments, in order to avoid circulating miRNAs contamination and evaluate their biomarker features in NSCLC.
The discovery of alterations in the EGFR and ALK genes, amongst others, in NSCLC has driven the development of targeted-drug therapy using selective tyrosine kinase inhibitors (TKIs). To optimize the use of these TKIs, the discovery of new biomarkers for early detection and disease progression is mandatory. These plasma-isolated exosomes can be used as a non-invasive and repeatable way for the detection and follow-up of these biomarkers. One ml of plasma from 12 NSCLC patients, with different mutations and treatments (and 6 healthy donors as controls), were used as exosome sources. After RNAse treatment, in order to degrade circulating miRNAs, the exosomes were isolated with a commercial kit and resuspended in specific buffers for further analysis. The exosomes were characterized by western blotting for ALIX and TSG101 and by transmission electron microscopy (TEM) analysis, the standard techniques to obtain biochemical and dimensional data of these nanovesicles. Total RNA extraction was performed with a high yield commercial kit. Due to the limited miRNA-content in exosomes, we decided to perform retro-transcription PCR using an individual assay for each selected miRNA. A panel of miRNAs (30b, 30c, 103, 122, 195, 203, 221, 222), all correlated with NSCLC disease, were analyzed taking advantage of the remarkable sensitivity and specificity of Real-Time PCR analysis; mir-1228-3p was used as endogenous control and data were processed according to the formula 2–ΔΔct 13. Control values were used as baseline and results are shown in logarithmic scale.
De lage overlevingskans in NSCLC (niet-kleincellige longkanker) van de patiënten is voornamelijk te wijten aan de beperkte effectiviteit van behandelingen in gevorderde ziekte en slechte vroege opsporing 1. Activerende mutaties in het EGFR-gen, die zijn ontdekt in een subpopulatie van NSCLC patiënten is bekend dat gevoeligheid voor kinaseremmers (TKI) tyrosine verlenen. Helaas zijn de meeste EGFR NSCLC gemuteerd patiënten ontwikkelen resistentie TKI 2. Vooral in dit verband een juiste diagnose is verplicht. In het algemeen, door lokalisatie van tumoren, verkregen biopsie weefsel in NSCLC is niet altijd uitvoerbaar. Daarom verscheidene studies worden uitgevoerd dat de niet-invasieve methoden om deze biologische gegevens te verkrijgen. Exosomes beschreven als vloeibare biopsie componenten die kunnen worden onderzocht om diagnostische en prognostische waarden met niet-invasieve technieken 3 te verkrijgen.
Exosomes zijn nanovesicles (40-100 nm diameter) van de endocytic oorsprong die zijn uitgebracht door verschillende celtypes, zowel in fysiologische en pathologische omstandigheden 4. Ze kunnen dragen eiwitten, vetten, mRNA en microRNAs. Bovendien suggereren verscheidene studies een pleiotrope rol van tumor afgeleide exosomes, die de groei en overleving van tumorcellen, angiogenese, en stromale extracellulaire matrix remodeling en geneesmiddelresistentie 5 kunnen beïnvloeden.
MicroRNAs (miRNAs) kort niet-coderende RNA die betrokken zijn bij post-transcriptionele regulatie, binding van het 3'-UTR van het doelwit mRNA en leidt het mRNA afbraak of een niet-vertaling 6. Selectief sorteren van miRNAs in exosomes is beschreven. In dit verband onlangs aangetoond in vitro en in vivo, een selectieve verpakking van miR-21 (een bekende miRNA met oncogene effecten) in exosomes vrijgegeven door chronische myeloïde leukemie cellijnen na behandeling curcumin 7.
Deexosomaal miRNA profiel is vergelijkbaar met het miRNA profiel van de primaire tumor en deze functie kan worden benut vroege diagnose en prognose 3. Concreet bestaat de mogelijkheid te karakteriseren door real-time PCR, geselecteerd miRNAs gecorreleerd met de moleculaire profiel van de ziekte, bijv., EGFR mutaties oa 8.
Hier is de analyse van een panel van geselecteerde NSCLC-gecorreleerde miRNAs 8-9 beschreven die zijn geïsoleerd uit exosomes vrijgegeven in het bloed van patiënten met NSCLC. Na het verkrijgen van de toestemming rapport op de hoogte van de patiënten, plasma van 12 NSCLC patiënten en 6 gezonde controles, werden geanalyseerd. De exosoom isolatie werd uitgevoerd met commerciële kit volgens het protocol van de fabrikant. Voordat exosoom isolatie, werden plasmamonsters behandeld met RNAse, teneinde verontreiniging circulerende miRNAs degraderen. We besloten om deze analyse met een commerciële kit voeren vanwege de beperkte normalisatie van andere tech-wachtrijen (bv., ultracentrifugatie) wat vooral van belang bij het werken met klinische monsters. Deze kit bevat een Proteinase K behandeling, die de RNAse zal degraderen, en vermindert zo het risico op exosomaal miRNAs degraderen na exosome lysis. MicroRNA analyse werd uitgevoerd door gevoelige en specifieke real-time PCR-analyse; mir-1228-3p werd gebruikt als endogene controle en de gegevens werden genormaliseerd volgens de formule 2 – ΔΔct. Controlewaarden worden als basislijn en resultaten worden getoond in een logaritmische schaal.
Met deze gecombineerde protocol was het mogelijk om een analyse uit te voeren exosomaal miRNA in plasma van patiënten met NSCLC. Deze gegevens kunnen de ziekte status weer te geven, maar dit dient te worden bevestigd door verdere studie.
